Application -

The application also serves as the Wait List Form. It should be submitted with the application fee at the time the child visit is scheduled.

Tuition Agreements -

Emergency Form -

 Emergency forms for all children are required before attending class.
Health Form -

Health forms with complete immunization records are required for attendance unless we have a completed Religious Exemption form on file.

Religious Exemption -

 This form is required for attendance if a child hasn't received immunizations for religious reasons.

Dispensing Medicine -

 This form authorizes staff to dispense medicine to a student.
